Participant Waiver, Release of Liability, and Parent/Guardian Consent
The Woodstock Police Service will do its best to provide a safe and enjoyable environment for all participants during the 2026 Kids & Cops Fishing Derby. However, participation in this event involves inherent risks that may result in injury, illness, property damage, or, in rare circumstances, death. This Waiver, Release of Liability, and Parent/Guardian Consent Form must be read, understood, and signed prior to participation.
WAIVER AND RELEASE OF LIABILITY
In consideration of my child being permitted to participate in the 2026 Kids & Cops Fishing Derby (the “Program”), including all related activities, I, the undersigned parent or legal guardian, acknowledge, understand, and agree as follows:
I understand and acknowledge that participation in the Program involves inherent risks, dangers, and hazards, including but not limited to risks associated with outdoor activities, fishing equipment, water hazards, weather conditions, transportation, and interaction with other participants, which may result in injury, illness, disability, death, or damage to property.
I knowingly and voluntarily assume, on behalf of my child and myself, all risks associated with participation in the Program, whether known or unknown, foreseeable or unforeseeable, including those arising from the negligence of the Woodstock Police Service and its officers, employees, volunteers, agents, representatives, sponsors, and partners (collectively referred to as the “Releasees”).
I agree that my child will comply with all rules, instructions, and safety requirements established by Program organizers. I understand that failure to follow such rules may result in removal from the Program.
If I become aware of any significant hazard or unsafe condition affecting my child or other participants, I will immediately notify a Program official.
On behalf of myself, my child, my heirs, executors, administrators, assigns, and next of kin, I hereby release, waive, discharge, and hold harmless the Releasees from any and all claims, demands, actions, causes of action, damages, costs, expenses, losses, liabilities, or injuries of any kind, including death, property damage, or personal injury, arising from or related to my child’s participation in the Program, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
I understand and agree that this Release is intended to be as broad and inclusive as permitted by the laws of the Province of Ontario and that if any portion is held invalid, the remainder shall continue in full force and effect.
PARENT/GUARDIAN CONSENT AND INDEMNITY
I certify that I am the parent or legal guardian of the participant named below and have the legal authority to execute this document on their behalf.
I consent to my child’s participation in the Program and agree to indemnify and hold harmless the Releasees from and against any claims, demands, actions, liabilities, costs, or expenses arising from my child’s participation in the Program, except where prohibited by law.
I acknowledge that I have carefully read this document, fully understand its contents, understand that I am giving up certain legal rights on behalf of myself and my child, and sign it freely and voluntarily without inducement.
PHOTO AND VIDEO RELEASE
I, the undersigned parent or legal guardian, hereby authorize the Woodstock Police Service to photograph, record, and publish photographs, video recordings, and other media depicting the participant named in this registration, including their name and likeness, for use in print publications, websites, social media platforms, promotional materials, news releases, and other communications produced and/or distributed by the Woodstock Police Service.
I understand and agree that these photographs, videos, and recordings may be used without further notice or approval and may be reproduced, distributed, displayed, or otherwise used for lawful purposes related to the promotion of Woodstock Police Service programs, services, and community initiatives.
I hereby release and hold harmless the Woodstock Police Service, its officers, employees, agents, volunteers, and representatives from and against any and all claims, demands, damages, liabilities, costs, or expenses arising from or related to the use, publication, reproduction, or distribution of such photographs, videos, or recordings, including any claims relating to privacy, confidentiality, defamation, or rights of publicity.
I acknowledge that participation in the Program is voluntary and that neither I nor the participant will receive any compensation, royalties, or other consideration for the use of such photographs, videos, or recordings. I further acknowledge and agree that all photographs, videos, and recordings shall remain the property of the Woodstock Police Service.
